Declaration of Eligibility

All 4-H youth competing in any 4-H contest must turn in a Declaration of Eligibility (DOE) form BEFORE the event date.

This is for EVERY event, even if it’s on the weekend.

The only time we do not require these forms is in the summer when school is out.

This form must be signed by the school and a parent.

If the child’s grades are not passing or we do not receive the form, they will not be eligible to compete, just like with sports in school.

Completed forms can be faxed to our office at (254) 435-6231, hand delivered, or emailed.
